2008 WESTMINSTER CC LADIES MEMBER INVITATIONAL SIGN UP FORM

Date of Event:  Tuesday, July 22, 2008

Please complete this form with names and GHIN number of each Member and Guest in your foursome and forward with your check to Sue Dery, 54 Jordans Way, Leominster, MA 01453.  A GHIN number is strongly recommended!  (Please remember we don’t need your LOCAL number, just your GHIN number.)  If you do not have a GHIN number, please provide the following:  copy of acceptable handicap card from another club OR five original, signed, attested score cards for 18 holes played during the year or during the winter of 2007/2008.  If there are any questions, please contact Sue Dery at (978) 537-3401 or email graskimah@aol.com.

NOTE:  If your foursome is interested in betting on “skins” (gross pars or better), the fee will be $20.00 per foursome.  You may forward this amount with the entry fee to avoid waiting in line on the morning of the Invitational.   Mulligans may also be purchased now.
